Short answer
Period bloating comes from hormone shifts: Cleveland Clinic says estrogen makes the body hold water, and estrogen and progesterone can slow or speed the gut. It starts a week or two before bleeding and usually eases within a few days of it. In a 2014 survey of 156 healthy women, 62% reported bloating in the 5 days before their period. No trial we found shows a probiotic eases it.
Five things to know
- Estrogen causes water retention, and fluids can build up when estrogen spikes and progesterone drops; the same two hormones can slow down or speed up digestion. [Cleveland Clinic]
- Premenstrual symptoms start and can build in the 2 weeks before a period and ease after it starts; MedlinePlus puts their end at 1 to 2 days into the period. [NHS; MedlinePlus]
- In daily diaries kept by 62 healthy women over 765 cycles, bloating scores peaked on the first day of the period, not before it. [White et al., 2011]
- Less salt, plenty of fluids, smaller and more frequent meals and regular aerobic exercise are the steps Mayo Clinic, MedlinePlus and ACOG list, and an ob-gyn can prescribe a water pill if water retention is a major symptom. [Mayo Clinic; MedlinePlus; ACOG]
- In a company-funded trial in 80 healthy women, a Lactobacillus strain did no better than placebo on premenstrual water retention over three cycles. [Sato et al., 2023]
Why do you bloat before your period?
Hormones, mostly. Cleveland Clinic explains that estrogen causes water retention: "When estrogen spikes and progesterone drops, fluids can build up." Mayo Clinic calls hormone changes the likely cause of holding water before a period and says diet might play a role. The Office on Women's Health says researchers think premenstrual symptoms come in the days after ovulation because both hormones begin to fall if you are not pregnant.123
Not all of it is water. The same Cleveland Clinic page says estrogen and progesterone "can cause intestinal gas by either slowing down or speeding up how food moves through your gastrointestinal (GI) tract."1 A Cleveland Clinic article on bowel changes around a period points to prostaglandins as well: they act on the muscle of the uterus as it sheds its lining, and they can have a similar effect on the bowels, which brings more frequent stools and sometimes diarrhea.4
The hormone story has one gap worth knowing. In a year-long diary study of 62 healthy women, "Neither estradiol nor progesterone levels were significantly associated with fluid retention scores," though the hormones were measured only in each woman's first and last cycles.9
When does period bloating start?
In the second half of the cycle. MedlinePlus says premenstrual symptoms start 14 or more days after the first day of your last period, and the NHS says they start and can build in the 2 weeks before a period. Cleveland Clinic and the Office on Women's Health put the start at a week or two before; Mayo Clinic says many people notice bloating 1 to 2 days before.65732
ACOG uses a tighter window to define premenstrual syndrome: symptoms present in the 5 days before a period, for at least three cycles in a row, that end within 4 days after it starts.8 In a 2014 survey of 156 healthy women at hospital gynecology clinics in Winnipeg, Canada, 62% reported bloating in the 5 days before menses and 51% during them, and bloating was the gut symptom reported most often overall.10
How long does period bloating last?
Usually into the first few days of the period. MedlinePlus says premenstrual symptoms usually clear 1 to 2 days after the period begins, the Office on Women's Health says within a few days, and ACOG's definition has them ending within 4 days. Two diary studies put the peak on the first day of bleeding, not in the days before it.638911
The first study followed 62 healthy women through 765 cycles with a daily diary. Self-reported fluid retention, which the authors say women commonly describe as bloating, peaked on the first day of flow, at 0.9 on a 0 to 4 scale. The authors call the day-1 peak an apparent contradiction of the idea that bloating is premenstrual, and rate the degree as very mild.9
The second, a University of Florida study of 78 healthy women on birth control pills, rated gut symptoms daily for 5 weeks: abdominal pain, diarrhea and indigestion, a group that covers bloating, scored higher on day 1 of bleeding than on any other day.11

How do you get rid of period bloating?
Cut the salt, drink more, eat smaller meals and keep exercising: these are the steps Mayo Clinic, MedlinePlus, ACOG and the NHS list, and none of them puts a time on relief. For water retention that stays a major symptom, ACOG says an ob-gyn can prescribe a diuretic.26812
What the sources say to try for period bloating
| Step | What the sources say | Source |
|---|---|---|
| Eat less salt | "Eating salty foods might make you retain more water" (Mayo Clinic); reduce salt (ACOG); avoiding caffeine, salt and sugar in the 2 weeks before may lessen many symptoms (Office on Women's Health) | Mayo Clinic; ACOG; Office on Women's Health283 |
| Drink plenty of fluids | Water or juice, not soft drinks, alcohol or other drinks with caffeine: "This will help reduce bloating, fluid retention, and other symptoms." | MedlinePlus6 |
| Eat smaller, more frequent meals | No more than 3 hours between snacks (MedlinePlus); six small meals a day instead of three large ones (ACOG); every 2 to 3 hours (NHS) | MedlinePlus; ACOG; NHS6812 |
| Get regular aerobic exercise | At least 30 minutes most days, and not just on the days you have symptoms (ACOG); throughout the month (MedlinePlus) | ACOG; MedlinePlus86 |
| Cut back on caffeine and alcohol | ACOG says to avoid both; MedlinePlus lists drinks with caffeine and alcohol among those to skip | ACOG; MedlinePlus86 |
| Magnesium or vitamin B6 | Magnesium may help reduce water retention (ACOG, Mayo Clinic), though the Office on Women's Health calls its results mixed; vitamin B6 may help with bloating (Office on Women's Health); not much evidence that supplements work (NHS). Ask a clinician first | ACOG; Mayo Clinic; Office on Women's Health; NHS82312 |
| A water pill (diuretic) | By prescription, if water retention is a major symptom (ACOG); taking it with NSAIDs may cause kidney problems (ACOG, Mayo Clinic) | ACOG; Mayo Clinic82 |
Sources as numbered in each row, each read on Oct 18, 2026. The steps come from pages on premenstrual symptoms in general, bloating included.
ACOG says to talk with an ob-gyn before taking any product advertised for premenstrual symptoms, since most "have either not been tested or have not been proved to be effective."8
Can a probiotic help with period bloating?
Not on the evidence so far. No trial we found made period bloating its main outcome. In the closest one, 80 healthy women aged 25 to 40 took tablets of Lactobacillus paragasseri OLL2809 or a placebo for three menstrual cycles, and premenstrual water-retention scores fell by about the same amount in both groups. The company that makes the strain funded the trial, and all three authors were its employees.13
The numbers, for the 77 women in the per-protocol analysis: on the water-retention part of the Menstrual Distress Questionnaire, premenstrual scores went from 4.1 to 2.9 on placebo and from 4.2 to 2.8 on the strain, and the gap between the groups was not statistically significant. In the authors' words, "No significant differences were observed between the groups in terms of physical symptoms, such as 'lower abdominal cramps', 'skin disorders', and 'swelling'." The differences they did report were in psychological measures, such as premenstrual irritability.13
A larger trial has not reported. A University of Florida trial, run with a probiotic company as collaborator, enrolled 187 healthy women on birth control pills to take a Bifidobacterium capsule (3 billion CFU) or a placebo daily for 8 weeks, to study belly pain and gut symptoms around their periods. Its main data collection ended in December 2021. As of Oct 18, 2026, the trial registry shows no posted results, and PubMed lists only its protocol.14 The other probiotic trials we found around periods enrolled women with painful periods, which this page does not cover.15
Physician's Choice, which publishes this site, makes Women's Probiotic, 50 billion CFU from 6 species in 1 capsule, and Vaginal Probiotic, 6 billion CFU from 3 coded strains in 1 capsule. Neither was tested in any study on this page. What to check on any women's probiotic label is in what is a good probiotic for women?17
See a doctor first if
you feel bloated regularly, or keep feeling bloated after changing what you eat, the NHS's reasons to see a GP, which adds that bloating that will not go away is sometimes a sign of something more serious; bloating comes with blood in your poo, another NHS reason to see a GP; it comes with vomiting, a stomach ache, a fever, or a swelling or lump in the tummy, for which the NHS advises an urgent GP appointment; you keep having water retention every month, Mayo Clinic's point to talk with your healthcare professional; premenstrual symptoms affect your daily life or lifestyle changes have not helped; or this line from the publisher of this site fits you: "If you are pregnant or breastfeeding, are seriously ill, or have a weakened immune system, consult your healthcare provider before taking a probiotic or any other dietary supplement."16212
Bottom line
Period bloating is the body holding water as estrogen and progesterone shift, with the gut slowing down or speeding up too; it builds in the week or two before a period, tends to peak on the first day of bleeding and fades within a few days. Less salt, more fluids, smaller meals and exercise are the sources' first steps, and no trial we found shows that a probiotic eases it.
